Thank you for your request for information about your accident which occurred at a construction site at The Priory School, Bury St Edmunds on 10 July 2017.
Your request was received on 12 September 2017 and I am dealing with it under the terms of the Data Protection Act 1998 (the Act).
The incident was reported but has not been investigated by the Health and Safety Executive. In these circumstances HSE is able to provide a copy of the incident report form, which is your personal data for the purpose of the Data Protection Act 1998. This is attached.
Certain personal data relating to third parties has been redacted (edited out) as it is exempt from disclosure under section 40(2) (personal information) of the FOI Act.
