Background: Acting on behalf of your department David Griffiths recently told a court that the death of a disabled woman who took her own life after her benefits were removed was not part of a widespread “systemic” problem.

FoI request: Given that your department has admitted to destroying its own investigations into various claimant deaths, if it exists, please kindly supply the TITLE, AUTHOR and DATE of documented evidence used by Mr Griffiths to support his statement above to the court subject to the key exclusions below.

Key exclusions: Names of claimants and everything related to the individual who this particular court case is about.
