Dear Haider Medical Centre

I would be grateful if you could supply the following information as of today‘s date:

1/ the number of patients registered at your GP’s practice;

2/ the number of patients whose records have had the objection code 9Nu0 (SystmOne XaZ89) entered, following the patient declining their consent to their clinical data being extracted electronically;

3/ the number of patients whose records have been coded with 9Nu4 (SystmOne XaaVL) forbidding any of the patient's data, from any NHS source, leaving the Health and Social Care Information Centre (HSCIC);

4/ the number of patient subject access requests (Data Protection Act 1998, section 7) received in 2014;

5/ fees quoted for subject access requests in 2014;

6/ number of patients, to date, who have registered for your online service - booking appointments, receiving medication lists and making repeat prescriptions.

Dear Laura Marks

The Freedom of Information Act 2000 is successfully being used to obtain information from hundreds of GP practices throughout England and Wales about data protection of health records for statistical analysis.

A request for information was sent to Laura Marks at Haider Medical Centre via WhatDoTheyKnow.com on 6 October 2015. Your response is long overdue. By law, under all circumstances, the practice should have responded by now.

My email was definitely received by your email server as has been demonstrated by examining log files.

I am emailing you separately and attaching a copy of the internal review procedure issued by the Ministry of Justice to which by law you must comply should I decide to initiate this process.

I am, however, using WhatDoTheyKnow to send you the FOI request again. The timescale to respond to this request (a maximum of 20 working days) will commence as soon as you receive this email - proof of delivery, if needed, can be obtained through log excerpts. When you respond, the FOI request of 6 October 2015 will be withdrawn.

Following is part of a message faxed and emailed to Laura Marks, Practice Manager, separately on 10 January 2016:
"Our server received a 250 Ok response from Haider Medical Centre's mail server timed 2015-10-06 11:36:14 at esg.NHS.net confirming delivery of this request which was given an id of 19C5C4B8185 by your system.
"If there is a problem, your IT team may be able to investigate based on the precise time and the id quoted above shown in the server log. I would be grateful if you could inform me if anything in the system is found which might explain your nil response."
A date was set after which, if no response is received or if the response is unsatisfactory, an internal review will be requested.
