Dear Crown Prosecution Service,

Please can you inform me of the following;

1) How many data loss/breach reports have been reported to the Information Commissioners Office by the CPS to include a breakdown the numbers associated with the CPS areas (whether geographic or specialist) and the nature of the loss (video interview discs, CCTV discs, statements, laptops etc). Please provide from 2012 to present.

2) How many incidents of data loss (evidence or otherwise) have been reported in the West Midlands CPS region from 2012 to present. Please include a breakdown of the nature of the data loss, which agency reported it and the outcome of such.

3) Please provide your policy document that shows the internal investigation that must be done should a prosecutor or such lose a piece of evidence, what procedures must be followed.
