Dear Monitor - Independent Regulator of NHS Foundation Trusts,

I would like to make a request under the freedom of information act.

I am aware that Monitor requests data from trusts on areas such as finance, workforce, activity - both current data and planned figures.

Please can you give me details of what data has to be submitted by a foundation trust to Monitor and at what frequency. I would request that I am sent a copy of each blank template used for data submission e.g. excel spreadsheet so that I can see exactly what data requested and how it is formatted.
