Data breaches/staff disciplined
With reference to the provisions of The Freedom of Information Act 2000, I would be obliged if you could provide me with answers to the following question(s):
1. Can you please provide me with the annual figures for the number of staff you have disciplined for data breaches in 2010/11, 2011/12, 2012/13, 2013/14 and 2014/15 so far?
2. Can you also please provide me with the annual figures for the number of staff who have performed a data breach without being disciplined in 2010/11, 2011/12, 2012/13, 2013/14 and 2014/15 so far?
By data breach I mean the intentional or unintentional release of secure information to an untrusted environment.

You will see from that response that the information provided relates only to data breaches that have occurred since October 2012. Although we hold the information in respect of breaches in previous years, this information is not held on a central computerised record system but on a combination of manual and computerised employee files. To provide the information you have requested we would need to identify all employees who had been the subject of a disciplinary investigation, locate and retrieve their individual employment files and examine the relevant papers in order to identify the nature of the misconduct allegations in each case. This information would then have to be collated and analysed. The amount of work involved will exceed the 18 hours limit specified in the Act.
