Dear Lewisham Borough Council,

Can you please provide

1. Evidence of any safety audit on design of this cycle route, from Loampit Vale to Worth Way?

2. Evidence of your investigations following the crash that caused the death of Paul Hutcheson on 24.06.13?

3. Details of any other crashes involving cyclists or pedestrians on this stretch of road in the past five years?

4. Details of any measures taken by Council to remove hazard from apparently encouraging/requiring illegal cycling over the footway (no marked route) to join the cycle route from Loampit Vale. Riding on the footway being counter to s.72 HA 1835.

5. Details of any measures taken by Council to remove hazard from access and egress from/to Matalan site

6. Details of any measures taken by Council to remove hazard from access to bus depot.
