Dear DWP CEP Correspondence,

In your FOI 752 reply you said "I believe", which suggests you should have used your obligation to seek a clarification or that to subvert the request you chose to interpret the request as one requesting personal data, which it was not.

New requests for self same information and more.

That said, please also disclose the forms the specific individual was requested to sign, with all personal data redacted. It would be expected that these redacted forms would be the same one's the CWP Provider included a participant signature section and that these forms would be of a standard nature used with all of it's CWP participants.

Additionally, with regards the CWP Provider under discussion please email me all of the specimen template documents they use, within which is included CWP participant signature section. This latter part of my request does not rely upon disclosure of any personal data whatsoever, as was the case in my original request of 23 February 2015 https://www.whatdotheyknow.com/request/c...

Please also treat this email as a request for an internal review.

I am the Claimant from the letters & consent to this use of my information by Refuted. If the DWP has any further concerns they should contact me via information they already hold using the tribunal & JCP letter reference numbers to locate me on their system where I will answer your security questions & give my consent to the details of my case being stored & made public as recorded information for FOI use as long as all identifiable information is redacted with the exception of my name & references but should not include my National Insurance Number. I wouldn't have scanned & uploaded my letter if I didn't consent to it being made public & Refuted already has my permission to use it in this way.
