Dear Department of Health and Social Care,
I am writing to request information under the Freedom of Information Act 2000.
I would like to receive information regarding bids submitted under the Coronavirus Support From Business scheme: https://www.gov.uk/coronavirus-support-f...
Specifically I would like to receive:
1a) A copy of information submitted to the Coronavirus Support From Business scheme online form by the following companies, and, b) the date on which it was submitted.

Initia Ventures Limited
P14 Medical Limited
Randox Laboratories Ltd
Total Productivity Solutions
2) How many completed online forms were received through the Coronavirus Support From Business scheme web portal in total?
Please note, this FOI request follows on from FOI ref FOI-1253054. I have considerably narrowed the scope of my questions and this should allow you to ascertain and provide this information easily within the £600 (24 working hours) cost limit.
If this request is too wide or unclear, I would be grateful if you could contact me as I understand that under the Act you are required to advise and assist requesters. If any of this information is already in the public domain, please can you direct me to it, with page references and URLs if necessary.
