Copy of HSE Report into the Death of Mr Mark Mullins

Response to this request is long overdue. By law, under all circumstances, Health and Safety Executive should have responded by now (details). You can complain by requesting an internal review.

Dear Health and Safety Executive (HSE),

In terms of the Freedom of Information Acts, please can you confirm:-

1] In light of the GROWING number of AVOIDABLE deaths, and CONTINUING ALLEGED DEPARTMENT-OF-WORK-AND-PENSIONS-DEATH-TOLL as per my earlier FOI requests to the HSE, that you the HSE will be launching a proper and full inquiry into the circumstances surrounding the AVOIDABLE death of Mr Mark Mullins as reported in the Coventry Telegraph newspaper website?

2] Please confirm that you, the HSE will, under the Freedom of Information Acts, supply a copy of your HSE Report into this tragic death immediately upon publication?

3] Please can you confirm what protocols you, the HSE have in place for investigating allegations of corporate manslaughter and/or culpable homicide? Especially when it is continuing, unabated and with the HSE allegedly standing idly by whilst the most vulnerable in our society are killed off?

The HSE have FAILED to respond in an effective manner on any level with this, or the associated requests.

The matter of HSE inaction is so serious and so MANY people have died without the HSE being evidently interested at all, that this issue is being audited in terms of a formal complaint under the following legislation as per Crown Prosecution Service Guidelines....
