Dear Health and Safety Executive,
In terms of the Freedom of Information Acts, please can you supply a copy of the Health & Safety Executive (HSE) Report into the tragic and avoidable death of 5 month old child, Kayjah Pardo, as reported in The Daily Mail newspaper website...
9th November 2011
Dear Health and Safety Executive (HSE),
Please pass this on to the person who conducts Freedom of Information reviews.
I am writing to request an internal review of Health and Safety Executive's handling of my FOI request 'Copy of HSE Report into the Death of Kayjah Pardo'.
The grounds for my making this request for an INTERNAL REVIEW are that the statutory time limit under the Freedom of Information Acts has now expired, and the Health & Safety Executive have now broken the law in this regard.
A full history of my FOI request and all correspondence is available on the Internet at this address:
Dear Mr McLean,
I answered this request for information on 6 December 2011 as part of a
combined response to the ten requests that you had made and it is
published under your name on the 'What Do They Know' website. In it I said
that HSE did not hold any information.
I do not intend to undertake an internal review, but if you are not happy
with my decision, please contact the Information Commissioner's Office.
