Please provide me with an electronic copy of the full submission to Care Quality Commission in respect of its two licence conditions:

1. The registered provider must ensure by 30 April 2010 that all medical and non medical equipment has been risk assessed and only equipment that has been fully maintained and assessed as safe to operate is in continued use. The registered provider must ensure that by 30 April 2010 it has a record of all equipment in use with appropriate maintenance and review dates for continued use.

2. The registered provider must ensure that by 30 April 2010, governance and audit systems to assess and monitor the quality of service provision are in place across all services. Evidence must be available to demonstrate this from 30 April 2010.
