I would like to access the following information and documents in your possession:

1. What is the job description for the role of an ICB Local Care Programme Delivery Manager in NHS North West London? This includes: responsibilities, duties, education, qualifications, experience etc.

2. Please release a full copy of the last contract(s) covering 2023 between the ICB and Opcare Ltd. for provision of NHS wheelchairs in Harrow & Hillingdon. This includes all relevant annexes describing the types of wheelchairs provided under the contract (attendant-propelled, self-propelled, TIS wheelchairs, powered wheelchairs etc.), the exclusion and eligibility criteria as given by NHS England and by the ICB, any annex to the contract specifying the wheelchair service provider’s obligation to appoint a Medical Devices Safety Officer, Data Protection Officer, the wheelchair service provider’s obligation to respond to FOI requests and subject access requests etc.

3. Please release a full copy of the last contract(s) covering 2023 between the ICB and AJM Healthcare for provision of NHS wheelchairs in Hammersmith & Fulham and in all other locations covered by AJM in the NW London ICB area. This includes all relevant annexes describing the types of wheelchairs provided under the contract (attendant-propelled, self-propelled, TIS wheelchairs, powered wheelchair etc.), any exclusion and eligibility criteria as given by NHS England and by the ICB, any annex to the contract specifying the wheelchair provider’s obligation to appoint a Medical Devices Safety Officer, Data Protection Officer, the wheelchair provider’s obligation to respond to FOI requests and subject access requests etc.

4. How much money was paid by the ICB to AJM Healthcare and Opcare Ltd. for provision of NHS wheelchairs in 2023?

5. How many service users in the NW London ICB received an NHS wheelchair in 2023 from Opcare Ltd and AJM Healthcare?

6. How many active wheelchair service users were registered with AJM Healthcare and Opcare Ltd. for the calendar year 2023?
