Continuity of Care assessments

Waiting for an internal review by Wye Valley NHS Trust of their handling of this request.

Dear Wye Valley NHS Trust,

Can you please tell me exactly how many Continuity of Care (checklist) Assessments have been carried by staff at the Hospital during the following years;

2008, 2009, 2010, 2011, 2012,2013

Who was responsible for carrying out these assessments in the relevant years?

How many of theses assessments were successful in being sent to the the PCT/CCG for a full assessment?

How many Continuity of Care cases have resulted in a successful NHS funding of Care package?

How many appeals have their been against a refusal to carry out a Continuity of Care Checklist Assessment?

How many appeals have their been against a decision not to award an NHS Continuity of Care Package and funding?

Dear Mrs Clark,

In your response you write to inform me that you have been directed by someone to give me the answer that you do not hold the information I seek . Can you please confirm who is directing you please?

Can you also confirm that you do not hold any information about the Continuity of Care (checklist) assessment because you should do, as your organisation is responsible for carrying them out. It is therefore fair to believe that this information is held by you, not the CCG.

However if I am wrong and you do not hold this information, can you please pass this matter on to the Herefordshire Care Commissioning Group for them to deal with.

I am writing to request clarification further to your internal review of Wye Valley NHS Trust's handling of my FOI request 'Continuity of Care assessments'.

What is the name of the Head of Nursing you refer to?

As previously stated I want details of the initial checklist assessments that are completed in the Hospital, by your staff as the CCG have stated that you should have this information as you are the originator of it.

If you do not collect this data, can you please admit it, instead of prevaricating and deliberately trying to mislead me and other members of the public.
