I would be very grateful for an answer to the following:

1a) Has Anyone been eligible for NHS CHC who had an assessed level of need across the 12 domains that was equal or less than the following:

2x high level of need
5x moderate level of need
3x low level of need
2x no level of need

1b) How many or what percentage were the same or below this level of need? Please give time period used to assess this.

2) what has been the lowest assessed level of need that has determined someone to be eligible for NHS CHC? (For example 1x high, 6 moderate, 3x low and 2x no needs).

This can be over a limited period of time depending on the budget available to access this information. Preferably within the past year. Please provide dates, many thanks.

3a) Please provide examples of health care needs which would be considered in the 12th domain?

3b) would any of the following be considered for domain 12: spasticity, contractures, oedema

Freedom of Information – Coronavirus Update 

Our response to Freedom of Information Requests (FOIs) may take longer for
us to complete at the moment because of COVID-19 implications. Freedom of
Information staff can work productively from home but are mindful that
many CCG staff are involved in crucial work to support Banes, Swindon and
Wiltshire residents through this difficult time, and this may impact their

are unable to extend statutory timescales, they will advise people that

during the COVID-19 pandemic.

prioritise essential services to ensure the wellbeing of our residents,

ensure that we comply with any changes or amendments to information rights
