My FOI request refers to the Berkshire Connected Care Project.

That is, the database comprising of information collated from GP surgeries in:

Newbury and District CCG
North and West Reading CCG
South Reading CCG
Wokingham CCG
Windsor, Ascot And Maidenhead CCG
Slough CCG
Bracknell and Ascot CCG

and non-GP data from The Royal Berkshire NHS Foundation Trust, Frimley Health NHS Foundation Trust, Berkshire Healthcare NHS Foundation Trust, Slough Borough Council, Reading Borough Council, and the four other local authorities in Berkshire.

I would like some updated information about this project, which I shall refer to as "CC" for ease in this FOI request, if I may, now that the database is live.

I requested this information a year ago, but you were unable to respond to any satisfaction, as the database had not yet gone live.

Now that it has gone live, I am asking the same questions.

Please could you provide me with the following:

• Could you kindly tell me who the data controller is for the Connected Care shared record database (e.g. is it NHS SCWCSU?)

• Could you kindly tell me which GP surgeries, and in which CCGs, are uploading information from their records to CC

• Could you kindly tell me the names of all other organisations that are extracting and uploading information to CC (presumably social care records)

• Could you kindly tell me whether the presence of any of the read codes 9Nd1, 93C3, 9Ndo, 9Nu0 or 9Nu4 would automatically, and completely, prohibit extraction and uploading of personal confidential information to CC from GP records?
• If so, which read codes?

• Could you kindly tell me whether combined information held in the Connected Care database is being, or is planned to be, used for secondary purposes (such as risk stratification, healthcare planning, research or commissioning)?
• If so, please could you tell me precisely what the effect of the 9Nu0 read code (Type 1 objection) and the 9Nu4 (Type 2 objection) is on any secondary processing of uploaded data by Connected Care, if that objection is present in a non-dissenting patient's GP record?
• And if so, is explicit, unambiguous, clear and affirmative, consent sought, or to be sought, from individuals, by the data controller, before their personal confidential data is processed for secondary purposes?

I would be grateful if you would be kind enough to send me the requested information promptly and in any event not later than the twentieth working day following the date of receipt of my request.

I would be grateful if you would kindly acknowledge receipt of this request as recommended by the ICO (“It would be good practice to acknowledge receipt of requests and to refer to the 20 working day time limit, so that applicants know their request is being dealt with”).
