PLEASE DISREGARD THE ABOVE REFERENCE NUMBERS WHICH ARE INCORRECT, I HAVE NO ABILITY TO EDIT THEM
Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
If you look at the "What do they know" website you will see that on 4 May I sent you a request entitled "Complete current policies, processes, procedures and guidance with visibility of linked information". Your systems sent an automated response to this confirming your receipt, also 4 May.
Yours sincerely,
Miss B White
Dear Miss White
I apologise that we did not receive this request.
We hold over 150 policies and these are all published to the AWP website.
Regards
Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
I am unhappy with this response.
AWP did receive the request and your automated system acknowledged that fact, there has been an error at your end.
I have asked for policies complete with links as the links on the online policies do not work.
There are policies e.g. ambivalence that are quoted in practice but which I cannot find on your website so I am uncertain whether they exist.
Please conduct an internal review as I do not believe that you have met the requirements of the FOI.
Yours sincerely,
Miss B White
Dear Miss white.
I apologise again that we did not receive this request. We will be reviewing internally the reason this did not reach our email inbox.
However, our response would have been to direct you to the external website for policies. We cannot extract over 150 policies and send them individually.
Please could you tell me which policy you were interested in and we can review

Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
I do not want 150 documents, I have asked for: "Please provide copies of ..... on the following subjects". Please see original question. Please let me know what timescales I can expect the information.
Thank you. I welcome your response.
Miss B White
Dear Miss B White
This request is exempt under Section 21 – information already reasonably accessible.
However, if you can specify which policy's link did not work we will endeavour to find that information for you

Dear Avon and Wiltshire Mental Health Partnership NHS Trust,
Please pass this on to the person who conducts Freedom of Information reviews.
I am writing to request an internal review of Avon and Wiltshire Mental Health Partnership NHS Trust's handling of my FOI request 'Complete current policies, processes, procedures and guidance with visibility of linked information'.
I have specifically asked for documents that contain links that do not work or where I cannot find the document. I believe you are not answering my request because you failed to answer the question in time and you are now hiding behind a clause in the FOIA as a reason not to give me the information. This information should be fairly easy for you to obtain.
A full history of my FOI request and all correspondence is available on the Internet at this address: https://www.whatdotheyknow.com/request/c...
Yours faithfully,
Miss B White
Dear Miss White
We have reviewed your request which is exempt under section 21.
The list we received related to variety of different subjects. If you could please send specific titles of the policies you feel are not working, we will then review and amend

Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
you are being obstructive.
In the original request I have said quite specifically " I note that you publish some of your policies on your website but the links don’t work. I also wish to see specific information that I have not found on your website."
"If there have been changes to any of these documents over the last 2 years please provide all versions or full details of the changes."
I am trying to access information that is not readily available on your website.
Please comply.
Yours sincerely,
Miss B White

Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
Thank you for telling me where to find the policies that are visible on your website. However, as per previous conversations, this does not show me about procedures and guidelines etc. And so you have not answered the original question.
For example, the links in the CPA policy do not work, I will paste in only 4 of numerous examples:
http://ourspace/ClientServices/CareProgr... called Mental Capacity act in this sentence: "Practitioners need to be aware that systems other than CPA may apply to particular service user groups such as MAPPA, S117 arrangements, Mental Capacity Act (MCA) and child protection arrangements". P6.
http://ourspace/Skills/ClinicalToolkit/P... called Risk Management on p5
"Every person referred to AWP will have a risk assessment completed and recorded in RiO. Procedure to adhere to in the management of risk can be found here" http://ourspace/ClientServices/CareProgr... p7
"Where a safeguarding issue has been identified of a sufficient degree to require raising concerns to a partner agency, procedures on the following link will be followed: Safeguarding" http://ourspace/ClientServices/PPS p7:
And so it goes on.
Yours sincerely,
Miss B White
Dear Miss White
I have included several documents linked to each of those policies in the zip file, and have now attached some further guidance.
This request is now likely to take us over the allocated time for each FOI.
Therefore if you have a specific request regarding a specific item, please send via the FOI system

Freedom of Information Team
Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
ref FRI 1974
So far you have sent me:
* a response letter
* "Additional documents" 3.7MB zip file, posted twice
* "further documentation" zip file
* the answer to a completely different FOI.
Initially you said that the information was publicly available.
Now you say that the information will take you too long to give me and will exceed the time limit.
AWP holds this information electronically on "ourspace", which is a database/ storage system for your policies and procedures. I wish to see this information electronically. There is nothing particularly time consuming about creating the response to this request. There are no sums to do, names to redact, no people to negotiate with to extract the information. Thus I cannot understand what is taking so long that you will need to charge me to respond to the request, or require me to start off a new FOI with the question so that you have longer to create the answer.
If it is too difficult and time consuming to save the individual documents from OurSpace please attach the entire database and I will be satisfied to find the documents linked from your policies for myself.
I have reported this case to the ICO. It has taken you too long and there is no reason I can see for you continuing to refuse to provide me this information under the FOIA.
Yours sincerely,
Miss B White
Dear Miss B White.
We have sent a huge amount of information relating to your request.
The Ourspace site is the Trust intranet site which is not available to the public.
We have asked that if you felt there was an item missing, to please submit a further request.

Freedom of Information Team
Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
I would be grateful if you can answer this part of the original question prior to me following your recent advice. You have not yet provided any information about which policies, procedures or guidance AWP use relating to 3 items.
1. Providing treatment showing thresholds or assessments used to determine whether therapy ( eg psychotherapy, DBT, MBT, etc) will be offered. I am interested in patients demonstrating suicidality, ideation and behaviour.
2. Ambivalence
3. Treatment of self harm.
I will, as you suggest in your response to RFI-1974 to this FOI of 4/5/2020, write a new FOI asking for specific links that are published on your website so that I can understand your published policies.
thank you
Miss B White
Dear FREEDOMOFINFORMATION (AVON AND WILTSHIRE MENTAL HEALTH PARTNERSHIP NHS TRUST),
There are 2 parts to this letter.
1. Where to find the response to the part of this question that are answered.
2. A note of the part of this question that are unanswered.
In response to my FOI of 4/5/20 which you called RFI 1974 you requested I create new smaller FOIs to get the information I required. As a result I created 7 new FOIs. Each of these separate FOIs appears on the website: “what do they know”.
Both you and the public will see that you have replied to all 7 FOIs in the response to #1 in the link I have pasted below. I have now understood how you have answered. As a result I have cross referenced the replies and have pasted a link to your answer in each of the 7 FOI questions.
The response to this FOI is given at this link: https://www.whatdotheyknow.com/request/l...
1. AWP CPA and risk procedures and guidance from the policy - answer at above link 1974-1 letter and zip
2. Access to Mental Health Care Assessment and Treatment Policy– answer at above link 1974 2 letter
3. Incidents policy p057 - answer at above link - letter 1974 4
4. risk assessment policy - answer at above link – letter 1974 5
5. Learning from deaths policy p173 - answer at above link- letter 1974 4
6. Induction Policy - answer at above link – letter 1974 6
7. Safeguarding adults at risk Policy - letter 1974 7
The following information from RFI 1974 (https://www.whatdotheyknow.com/request/c...) is still unanswered.
“You have not yet provided any information about which policies, procedures or guidance AWP use relating to 3 items.
1. Providing treatment showing thresholds or assessments used to determine whether therapy ( eg psychotherapy, DBT, MBT, etc) will be offered. I am interested in patients demonstrating suicidality, ideation and behaviour.
2. Ambivalence
3. Treatment of self harm.”
Yours sincerely,
Miss B White
