The appropriate limit is specified in the Freedom of Information and Data
Protection (Appropriate Limit and Fees) Regulations 2004 and for an NHS
Trust it is set at £450. This represents the cost of one person spending
2½ working days in determining whether the Trust holds the information,
and locating, retrieving and extracting the information. Consequently, the
Trust is not obliged by the Freedom of Information Act 2000 to respond to

We are unsure of your definition of a Serious Case Review, and are
therefore providing the number of Serious Untoward Incidents (SUI)
investigated in 2011 - 13. If your definition of a Serious Case Review is

11 of the SUI’s investigated are now complete with 2 still under review.

Management Plans & Finance

 

9.    How Many Service users are on service plans, (per hospital within
your trust and please itemise short term or long term)

While we can confirm that we document and plan service developments, we
are unable to provide a figure regarding to future staffing levels,
firstly because this is opinion, secondly such information is commercially
sensitive (s.43) and thirdly to release such information may prejudice the
Trusts ability to conduct our affairs (s.36).

 

10. Policy of any form of harassment from staff to service users.

We have no policy statement referring to the harassment of service users
by staff. The documentation we have in this area deals with bullying and
harassment between staff members and where staff feel visitors or patients
have bullied them.

 

Staff are contracted to adhere to relevant professional codes of practice
and Trust policy on behaviour and system use. Any claims or complaints
raised by members of the public, or other staff members concerning
allegations of bullying or harassment of members of the public by staff
will be taken very seriously and investigated thoroughly.

 

11. Staffing costs over the last 5 years and forecast cost for next year
breaking down into front line staff (doctors nurses etc.) and Managers /
Admin

12. Policy on paging resuscitation staff when needed at times of high
service users.

We are unsure what your definition of high demand is. However we an
confirm that Trust does not have a policy on paging resuscitation staff in
times of difficulty as we aim to deal with all patients according to what
is best for them in their individual circumstances.
