"Two clinical organisational failures are not two one-offs: they point to an underlying systemic problem that may be latent in other units. It is vital that we recognise why, and what we can do about it."

The story seems to be getting even worse:

"A catalogue of maternity failings at Shrewsbury and Telford Hospitals NHS Trust were contained in a report leaked to the Independent on Tuesday.

Rhiannon Davies, who campaigned for an inquiry after the death of her baby, said 25 new families had come forward overnight.

The NHS declined to comment."

From the Daily Mail:

'Detectives last night opened a criminal inquiry into Britain's biggest maternity scandal.

They will investigate failings at two hospitals where dozens of babies died or suffered brain damage.

The dramatic development came after police met NHS officials and the leader of a probe into 1,200 cases dating back to the 1970s.

It means maternity staff at the Shrewsbury and Telford NHS trust could face criminal charges over a scandal whose shocking scale was exposed by the Daily Mail.'

'The largest ever review of maternity care in the NHS has revealed it is now examining nearly 1,900 cases.'

'"A number of individuals have described a culture of defensiveness, denial and/or lack of openness that existed at the time in the maternity service and trust more generally. While such a culture clearly does not excuse any actions or behaviours, it may help explain them," NHS Improvement concluded.'

Daily Mail:

'Standards are still falling at a hospital trust at the centre of a maternity scandal, the watchdog warned last night.

The Care Quality Commission said that poor care is the norm at Shrewsbury and Telford.

Inspectors singled out incompetent leadership and out-of-their-depth staff at the NHS trust, one of the country's most poorly rated. They found examples of nurses blaming each other for not completing tasks and workers turning a blind eye to poor care on their wards.'

Yet another national scandal which passed under the PHSO radar. They seem totally unable to spot patterns of systemic failure.
