Dear Northern Health and Social Care Trust (Northern Ireland),
I am requesting information as per the following, for Antrim and Causeway Hospitals, each shown seperate and broken down into monthly intervals for the years 2010, 2011 and 2012;
1. How many complaints have been made to the hospital regarding the accident and emergency department?
2. How many ongoing complaints/cases are the hospital currently handling regarding the accident and emergency department
3. How many deaths in accident and emergency have resulted in post mortems 2010, 2011, 2012
