& Maidenhead CCG, NHS Slough CCG and NHS Bracknell & Ascot CCG (the 3
CCGs).  Please note that the 3 CCGs have now merged and will operate as
one, called NHS East Berkshire Clinical Commissioning Group (CCG). For
