Dear Parliamentary and Health Service Ombudsman,

In an email to the National Audit Office, the CEO of the PHSO stated:

"We also have accountability to our Board, to whom we have recently written a detailed response concerning the latest challenges to our performance figures."

1. Please provide the written "detailed response concerning the latest challenges to our performance figures" provided to the Board and mentioned in the email.

2. Please provide all information concerning the other "challenges to our performance figures" that the email implies exist.

3. Please provide all communications between the PHSO Board and PHSO staff, including the Ombudsman and CEO, between 1 June 2020 and the current date.

Dear Parliamentary and Health Service Ombudsman,

Thank you for your reply. However you haven't specified the information you require.

To try to clarify:

1. I am requesting any emails, documents etc sent/presented to the Board by staff members, such as the Ombudsman and CEO. For example the Ombudsman's report to the Board of 17 December 2020 recently obtained by FOI request.

3. The request covers the period from 1/6/20 to 9/8/21.

I am writing to request an internal review of Parliamentary and Health Service Ombudsman's handling of my FOI request 'Communications between the Board and PHSO staff'.

On 15 November 2021 I was asked to clarify the information I was requesting and I replied with a clarification on the same day. I have not received a reply since I provided the requested clarification. This review request is predicated on the failure of the PHSO to reply to my clarification of the scope of the request.
