Commerce Street /Trentham Road Interchange and No Entry Signs in general

I note from recent media reports that there was an accident caused by a confused driver exiting left,at night, from the Trentham Road interchange(Stoke on Trent) into Commerce Street, which has been designated a One way Street.I have contacted the local council regarding the absence of `No Entry`signage at that junction with the interchange,when the preceeding similar junction off the same interchange has No Entry signage in place.There appear to be no warning signs that the entry into Commerce Street is forbidden but the Council advise me " Any development plan specific to the junction of Commerce Street and the Trentham Road
interchange is under the ownership of Highways England (HE), as this project was a design and build construction by them. Your request should be redirected to Highways England, who will also have the rational for installing the NO ENTRY signs at the top of the slip rounds only".
FOI Q1.When was the Trentham Road interchange project completed and were there any `No Entry`signs planned for the junction with Commerce Street?
FOI Q2.What is the Highways rational for installing the NO ENTRY signs at the top of the slip rounds only, and should there be any indicaton by way of road makings or signage that a road is `No Entry`?
FOI Q3.There have been several incidents of vehicles driving the wrong way down motorways after entering via the exit slip road.What is the current policy regarding signage at the mouth of motorway exit slip roads,and are any modifications to the policy in discussion or completion in the light of the recent incidents?
FOI Q4.There is regular confusion at the entrance to `bus gates`in city areas where the signs are not impactful.Is it possible for an authority to implement signs stating `No Entry`and `Buses and taxis only`(for example) rather than the current blue signage which is proven to be confusing and ineffective?I believe I have seen such a combination of No Entry plus` exemptions authorised`signs used in some locations.Please advise if my memory is true,and confirm which situations currently allow the combination.

The junction was completed sometime in 1997,over 10 years ago,apparently Staffs County Council oversaw the project,Highways have no copy of plans or requirement for No Entry signs, all other requests regarding better signage basically covered by `a matter of opinion`if required.
