COINS User Survey 2009, should take you about fifteen to twenty minutes to answer, please feel
free to make comments where appropriate.
The data collected from this survey will be treated anonymously, however there is an option at the
end to give contact details to enable strictly confidential follow up to any of your concerns or
comments.
All questions with an asterisk are mandatory, however to get the best response we would be grateful
if you could answer all questions as fully as you can. If you do not have time to complete the survey
in one session you can close the window and return to it at a later point by using the link in the e-
mail or on the COINS home page. The survey can only be completed once per computer.
