surgeries, as well as some specialised secondary care services. Therefore,
any recorded information NHS England may be able to provide in response to

commissions.

Dear FOITEAMCRMMAILBOX (NHS ENGLAND & NHS IMPROVEMENT - X24),

Having double checked your response l would be grateful if you could just clarify a couple of points, sorry if you had flagged this request as over.

1. I clearly requested a copy of the last submitted self-assessment made by Chesterfield Royal Hospital Foundation Trust (CRHFT) regarding "7 Day Hospital Services". In October 2019 you published the results of all the self-assessments (https://www.england.nhs.uk/publication/7...), CRHFT was in you list of results and as such may l ask why you are stating that you do not hold the assessment as one would assume you would need this in order to produce the results? If you were provided the results then may l ask who holds this data?

Yours faithfully

Dear Mr Kent,

Thank you for your correspondence dated 21 February 2020.

Your correspondence read:-

“1. Please could you provide a digital copy of the last submitted
self-assessment made by CRHFT regarding "7 Day Hospital Services".

2. Please could you list all self-assessments made by this CRHFT
submitted  in 2019 that are not exempt from public release.””

NHS England is not, at this stage, able to proceed with your enquiry under
the (FOI) Act, as your request is unclear.

It may help if we first explain that the FOI Act gives you the right to
ask any public sector organisation for the recorded information they have
on any subject. You can ask for any information you think a public
authority may hold. The right only covers recorded information which
includes information held on computers, in emails and in printed or
handwritten documents.

Your request can be in the form of a question, rather than a request for
specific documents, but the authority does not have to answer your
question if this would mean creating new information or giving an opinion
or judgment that is not already recorded.

We would be grateful if you could clarify which service you are interested
in with regard to question 2.

As set out at Section 1(3) of the FOI Act, when further information is
required in order to identify and locate the information requested, and
the applicant has been informed, no further action is required until the
additional information has been received.

Please send your clarified request to [1][NHS England request email].

Once received, NHS England will write to acknowledge receipt of your
request and to provide you with a reference number. Please note that the
20 working day timeframe for complying with an FOI request does not
commence until the above information is provided.

Please do not reply to this email. This message has been sent from a
central mailbox. To communicate with NHS England regarding Freedom of
Information (FOI) requests, enquiries or complaints we ask these are sent

Dear NHS England,
You have clearly misread this FOI all relating to Chesterfield Royal Hospital Hospital Foundation Trust and self-assessments submitted by them.

To easily read all of this thread use the following url: https://www.whatdotheyknow.com/request/c...

I originally asked the following two questions

1. Please could you provide a digital copy of the last submitted self-assessment made by CRHFT regarding "7 Day Hospital Services".

2. Please could you list all self-assessments made by this CRHFT submitted in 2019 that are not exempt from public release.

You replied to both of the above by saying you do not hold the information.

On the 21st February l asked for clarification as follows:

You now have asked me to clarify "which service you are interested in with to question 2"

Please supply the information as requested in question 1 i.e a copy of the 7 day hospital services self assessment made by CRHFT or provide the requested clarification as of 21st February 2020 if indeed you do not hold this data.

With regards to question 2, after processing question 1, l would like a list of ALL self-assessments made by CRHFT that are not exempt from public release. ALL means all irrespective of service, if its a self assessment made by CRHFT then l would like to see it on the list.

Regards

Mr. Kent

Dear NHS England,

I have tried to be patient given the current environment but you have still not replied to my FOI.

Dear FOITEAMCRMMAILBOX (NHS ENGLAND & NHS IMPROVEMENT - X24),

Yet another 20 working days have gone by without a response, can you please provide an update with your intentions regarding this FOI, please see my comments 7th June.

Yours faithfully,

Mr. Kent

Dear NHS England,

Please pass this on to the person who conducts Freedom of Information reviews.

I originally asked the following two questions about Chesterfield Royal Hospital Foundation Trust (CRHFT) regarding self-assessments that they completed and submitted to you but you have failed to provide adequate response.

1. Please could you provide a digital copy of the last submitted self-assessment made by CRHFT regarding "7 Day Hospital Services".

2. Please could you list all self-assessments made by CRHFT submitted in 2019 that are not exempt from public release.

You replied to both of the above intially by saying that you do not hold the information and then on the 21st February asked for clarification which l provided but still serveral months later l have had no response.

Please note with regards to question 1 in which you stated that you do not hold the data regarding the seven day services, this does seem a little odd as in October 2019 you published the results of all the self-assessments (https://www.england.nhs.uk/publication/7...). As CRHFT was in your list of results may l ask how you were able to produce and publish the results without having a copy of the assessment in which to obtain the results for your summations?

**THIS IS AN AUTOMATED RESPONSE**

Dear Customer,

Thank you for your email. We are currently receiving a higher than usual
volume of emails so it may take us a little longer to respond directly to
your email. In the meantime, you may find the following information
helpful.

Please note: our normal working hours are 08:00 to 18:00 from Monday to
Friday (excluding Bank Holidays). Emails received at the weekend will not
be reviewed until the next working day.
How can the Customer Contact Centre help me?
NHS England commissions or buys primary care services; for example, GPs,
dentists, opticians, and pharmacy services. We are also responsible for
prison healthcare, military health services, and some specialised
services. We can provide advice about accessing, giving feedback or making
a complaint about these services.
You may be able to find the answer you are looking for in our Frequently
Asked Questions<https://www.england.nhs.uk/contact-us/ho...>
and our website explains how to share feedback or make a
complaint.[1]https://www.england.nhs.uk/contact-us/co...
What if I have an enquiry or complaint about hospital care, NHS 111 or
out-of-hours GP services?
If your enquiry or complaint is about secondary care, this includes
hospitals, NHS 111, mental health services, out-of-hours services and
community services such as district nursing, you will need to contact the
organisation that provided the service. All hospitals have a Patient
Advice and Liaison Service (PALS)
[2]https://www.nhs.uk/common-health-questio...
. Alternatively, you can contact your local Clinical Commissioning Group
(CCG). You can find their contact details on the NHS
website[3]https://www.nhs.uk/Service-Search/Clinic...
Does the NHS England Customer Contact Centre provide medical advice?
No. Our advisors are not clinically trained and are unable to provide
medical advice. If you require emergency medical attention, dial 999 or
attend your local Accident and Emergency department if you are able to do
so.
If you require non-urgent medical advice please contact your GP, local
walk-in or urgent healthcare centre, or call NHS 111. Calls are free from
landlines and mobile phones.

How do I report a change of name or address?
You should report a change of name or address to your GP practice so they
can update their records. If your new address is outside the practice’s
catchment area you may be asked to register with a new GP practice.
You should also advise your dentist or optician of a change of name or
address as they keep their own records.
