Chemotherapy

Response to this request is long overdue. By law, under all circumstances, NHS England should have responded by now (details). You can complain by requesting an internal review.

Dear NHS England,

I would like to ask 2 questions regarding the administration of chemotherapy

1. Does the NHS receive any
Money for giving chemotherapy?

2. If so, then how much does the NHS or a individual PCT receive for this?

Yours faithfully,

Mike Brodie

If NHS does get paid to deliver chemotherapy does this money go to the regional trust or CCG responsible for administrating the treatment?
Or does it go to NHS England directly?

Dear Mr Brodie,

Re:      Freedom of Information request (Our Ref: FOI-022689)

Thank you for your Freedom of Information (FOI) request dated 25 October
2016.

Your exact request was:-

“1. Does the NHS receive any money for giving chemotherapy?

2. If so, then how much does the NHS or a individual PCT receive for
this?”

NHS England hold some relevant information relating to your request.

Chemotherapy care is commissioned and funded by NHS England, as part of
the wider portfolio of directly commissioned specialised services. These
services are delivered by a range of providers in England which are
reimbursed for both the treatment delivery and drug costs. However, the
payment mechanisms do sometimes vary depending on the hospital and for
some very high-cost chemotherapy drugs. The scope of specialised services
is contained within the Prescribed Services Manual – this can be found at:

·
[1]https://www.england.nhs.uk/commissioning...

Providers of chemotherapy services in England are paid directly by one of
the 10 NHS England specialised commissioning hubs. The administering of
chemotherapy is reimbursed at national tariff rates adjusted for the
Market forces Factor (2016-17 price bases):

[2]Tariff Outline

The total payment to an individual provider would depend on the level of
activity undertaken. High cost chemotherapy drugs are reimbursed at the
actual cost of the drug and other drugs plus procurement costs are
reimbursed at locally agreed values.

Below are the numbers from the exercise undertaken to analyse the
specialized commissioning expenditure by CCG using the 2015/16 data:

  £m
Chemotherapy Delivery 354.2
Drugs 1,328.8
Total 1,683.0

Please note that the expenditure figure quoted for drugs includes the
chemotherapy treatments agreed via the Cancer Drugs Fund.

We hope this information is helpful. However, if you are dissatisfied, you
have the right to ask for an internal review by writing to us, within two
months of the date of this letter, to:

Dear Mr Brodie,

Re:      Freedom of Information request (Our Ref: FOI-022689)

Apologies for the delay in our response.

Thank you for your Freedom of Information (FOI) request dated 07 December
2016.

Your exact request was:-

1.    Is this the payments that are given to the provider upon giving the
treatment?

 2. Can you please explain what these prices represent?
 3. Is it the drug manufacturer that provides the money to give to NHS
England & the 10 NHS England specialised commissioning hubs?

NHS England holds some information relevant to your request

Is this the payments that are given to the provider upon giving the
treatment?

Yes, but this only covers the cost of delivering the chemotherapy where
the patient has attended solely to receive chemotherapy treatment. The
cost of the drug and preparation of the drug is reimbursed separately. The
tariffs above  are the base tariffs and as with all other national tariffs
would be increased by set percentages (known as the Market Forces Factor
or MFF for short) to reflect local cost base differences. So Trusts based
in London would receive higher percentage uplift for MFF than providers
based in the North of the country. These percentages are nationally set
and range from 0% to 29.8%).If the patient is receiving chemotherapy
whilst they are an inpatient (very small % of patients) which may or may
not directly relate to chemotherapy, the provider is not separately
reimbursed for the chemotherapy element, as this is covered by the
inpatient tariff income they receive.

There is additional information about tariffs at the following web link;

[1]https://improvement.nhs.uk/resources

Can you please explain what these prices represent?

These are not prices, they are the total payments made to providers for
the delivery of chemotherapy and the associated chemotherapy drugs costs.

Is it the drug manufacturer that provides the money to give to NHS England
& the 10 NHS England specialised commissioning hubs?

No. NHS England Specialised Services receive a funding allocation from the
Department of Health which is then allocated to the 10 NHS England
specialised commissioning hubs who hold NHS contracts with the
organisations providing the care.
