Section 14 of the NHSI improvement guide for NHS chairs and non execs https://improvement.nhs.uk/documents/122...
states that normally chairs will generally serve a maximum of 8 years, with a maximum of 10 years in individual circumstances.

1. Please confirm that Sir Hugh Taylor was appointed chair in the first quarter of 2011, almost 9 years ago.

2. Please provide details of all board or board committee minutes where any decisions were made that extended Sir Hugh Taylor's period of office beyond 8 years.

3. Please confirm whether any decision has been made regarding when Sir Hugh Taylor's period of office will end.

4. Please confirm whether any discussions have been held with NHS England or Improvement, in relation to extending Sir Hugh Taylor's period in office beyond 10 years, and please provide dates and attendees of any meetings where this has been discussed.
