Dear Information, Freedom

You have failed to respond within the statutory time frame and you have not explained why.

This is one of a number of queries you are late to respond to and this is noted.

The request is not a complex one .It is a very simple question.

Failure to respond will result in yet another referral to the ICO . You have previously been served notices for failing to respond to other queries . It would be good to see Information Governance comply with the law otherwise confidence in services becomes undermined

Yours sincerely,

Alea

Good morning,

FOI 2019-077

Firstly, may we apologise for the delay in responding to your request.
The team who deal with Security and CCTV have not given their response in time, so I have asked the Interim IG Manager, Mr Gerard Kursten to follow this up and to carry out an internal investigation.

We hope to be able to reply fully in due course.

Dear Camden and Islington NHS Foundation Trust,

Please pass this on to the person who conducts Freedom of Information reviews.

I am writing to request an internal review of Camden and Islington NHS Foundation Trust's handling of my FOI request 'CCTV on mental health wards'.

Assurance that you are following this up without a defined deadline is not good enough. Statutory time limits are there for a reason.

A full history of my FOI request and all correspondence is available on the Internet at this address: https://www.whatdotheyknow.com/request/c...

Yours faithfully,

Alea

Dear Information, Freedom,

I request an internal review given the very very serious discrepancy between this response and the response to a SAR made in December 2018. You are reminded that the Duty of Candour applies and the Trust are considered to have breached it.

Given you have admitted there IS CCTV on Rosewood ward please explain why when a SAR request was made within one day of filming and a very serious incident occurring your team stated in response that Rosewood ward had NO CCTV. There was a formal complaint raised at the exact same time. As you are FULLY aware.

As you are also FULLY aware the SAR was made in response to a violent incident whereby a patient was injured and unlawful detention was alleged. Your response was challenged on multiple occasions as it was clearly untrue. Your team were also asked if the tape would be made available to the police. You denied any CCTV cameras existed and that there was NO filming at all on the ward.

So please review your response and it is suggested you alert your senior management that you have admitted to there being CCTV when having denied it in relation to a patient assault. This is a live Ombudsman investigation as you are FULLY aware. These discrepancies show up very serious honesty and transparency issues including the fact that you were willing to seemingly hide footage pertinent to a criminal allegation.

Refer this to Angela McNab the chief executive.

Which position is the truthful one?

Yours sincerely,

Alea

Dear Information, Freedom,

You have failed to respond as requested and required by law.

Your FOI response is totally contradictory to very recent information supplied BY YOUR OWN TEAM requested as part of a SAR.

So you are AGAIN asked to confirm that the information provided in this FOI is accurate according to the governing legislation.

It is of serious concern that a NHS information governance team would mislead in this way. However that may have occurred.

Yours sincerely,

Alea

Dear Information, Freedom,

You have failed to respond to the request of 28th May for an internal review

Please do so

Yours sincerely,

Alea
