Dear NHS West Suffolk Clinical Commissioning Group,

I would like to request the information about the following points around the CCG's decisions with patient care and its structure.
• What criteria does the CCG use to select which inhalers should be prescribed for Asthma and COPD?
• How does the CCG compare the cost-effectiveness , efficacy, safety and patient usability of different inhalers when selecting which COPD and Asthma inhalers should be included on its local guidance?
• Over the past three years, has the CCG introduced a planned programme of care which resulted in the medicines optimisation team, contractors or GP practices proactively reviewing patients and aligning their COPD and/or Asthma inhalers to alternative options?
• Please list the alternative asthma and COPD inhalers that were introduced by the CCGs medicines optimisation team, contractors or GP practices during any planned programme of care that proactively reviewed the use of asthma and/or COPD inhalers over the past 3 years
.
• Over the past three years, has the CCG used a QIPP scheme to introduce a planned programme of care which resulted in the medicines optimisation team, contractors or GP practices proactively reviewing patients and aligning their COPD and/or Asthma inhalers to alternative options?
• Please list the alternative asthma and COPD inhalers that were introduced by the CCGs medicines optimisation team, contractors or GP practices when using a QIPP scheme to introduce a planned programme of care that proactively reviewed the use of asthma and/ or COPD inhalers over the past 3 years?

I write further to your e-mail of 8 May 2018 regarding information held by
Ipswich and East Suffolk Clinical Commissioning Group and West Suffolk
Clinical Commissioning Group. Your request has been dealt with in
accordance with the Freedom of Information Act 2000.

               Ipswich & East Suffolk CCG responses are below.  West
Suffolk CCG responses are contained in the attachments.

 

 

                I would like to request the information about the
following points around the CCG's decisions with patient care and its
structure.

 

                • What criteria does the CCG use to select which inhalers
should be prescribed for Asthma and COPD?

 

                              The decision on which inhalers are included
on the Ipswich and East Suffolk formulary is made by the Ipswich Hospital
respiratory consultants in collaboration with the CCG.  Decisions are made
based on clinical evidence, patient usability factors, such as the device
and frequency of administration, side effect                       profile
and cost.

 

                • How does the CCG compare the cost-effectiveness ,
efficacy, safety and patient usability of different inhalers when
selecting which COPD and Asthma inhalers should be included on its local
guidance?

 

                              Ipswich Hospital respiratory consultants
lead this process. 

 

                • Over the past three years, has the CCG introduced a
planned programme of care which resulted in the medicines optimisation
team, contractors or GP practices proactively reviewing patients and
aligning their COPD and/or Asthma inhalers to alternative options?

 

                              Yes – during 2015/16 the CCG implemented a
Local Enhanced Service requiring GP practices to undertake medicine
optimisation reviews for patients with COPD.  Part of the review asked GP
practices to consider whether a formulary inhaler could be used instead of
a non-formulary or unlicensed inhaler.

 

                • Please list the alternative asthma and COPD inhalers
that were introduced by the CCGs medicines optimisation team, contractors
or GP practices during any planned programme of care that proactively
reviewed the use of asthma and/or COPD inhalers over the past 3 years.

 

                              COPD: 

                              Relvar Ellipta 92/22, Fostair

                              Seebri Breezhaler, Incruse Ellipta

 

                • Over the past three years, has the CCG used a QIPP
scheme to introduce a planned programme of care which resulted in the
medicines optimisation team, contractors or GP practices proactively
reviewing patients and aligning their COPD and/or Asthma inhalers to
alternative options?
