I can respond re part 5 of your request however:

 

Request

Your Document Retention Policy.

 

Response

We don’t hold a Document Retention Policy as such. We have a guidance
document we refer to and retention for all personal data is now held in
our Information Asset Register (IAR) which is being finished. Below is an
extract referring to data retention from our Data Protection Policy. There
will also be something similar in the revised Information Security Policy
which I am applying the Section 22 exemption to because it is ‘Information
intended for future publication’.

 

Retention of Data

Personal data must not be retained for longer than is necessary. DSFRS
Information Asset Owners and managers are responsible for ensuring that
the Record Retention Schedule within the Information Asset Register is
applied to all records and documents holding personal data, by having
regular or automated deletion or destruction of personal data in systems,
paper files and on network folders.

 

All documents containing personal data should be disposed of securely
using the Service’s confidential waste procedure.

 

Review

You have the right to ask us to review this decision.  If you wish to do
this please set out your grounds for review in writing and send them to
the Information Officer at the Service Headquarters address or e-mail
[1][Devon and Somerset Fire and Rescue Service request email].

 

Appeals

If you are still dissatisfied after a Service review you have the right to
appeal to the Information Commissioner at the Information Commissioner’s

I write in response to your FOI request below and attach the following
document as part of your request:

 

1.       A copy transcript of the Fire Ground Event & Decision Log (Ref
Fire ground incident log as per below). This has been redacted by applying
Section 40 (2)(a) in respect of any personally identifiable information
held within.

 

Responsibility for FOI has very recently transferred into the data
protection team (this week) and I am therefore bringing myself up to speed
with any documentation that we may have. The remaining documents require
consideration of exemption S36 by our qualified person within the Service
(the Monitoring Officer) who is not in the office now until after the bank
holiday weekend. Therefore, unfortunately this means that we are not in a
position to be able to fully respond to your request as per the deadline
set by the ICO in respect of case reference no FS50832220.

 

I apologise for this further delay in responding to your request but I
will endeavour to respond fully once the Monitoring Officer is in the
office. For your information, I have today contacted the caseworker at the
ICO to give the same explanation.

 

In the meantime, if I can be of any other assistance, please do not
hesitate to contact me.

FREEDOM OF INFORMATION ACT 2000

 

We have reviewed our previous disclosure ref: ‘ACFOI 01 Full Fire Control
Incident Log_Redacted’.  In line with the ICO’s guidance on ‘Requests for
personal data about public authority employees’, DSFRS have subsequently
taken the decision that the names of senior staff (Area Manager and above)
will be released.

We believe that an incorrect exemption has been applied in the case of
section 31 (2) (e) and have subsequently amended our redactions to show
section 36 (2) (c) as the correct exemption that should have been
applied. 

 

We have notified the ICO of this amendment in our response to them
following notification of your complaint (ref: FS50832220) received on
14th August 2019.  We have also attached the Public Interest Test
completed in support of our use of Section 36.

 

Please find attached a copy of the document with subsequent alteration to
the redacted information.

of the Cathedral Yard Fire Incident in 2016, please find attached the
following attachments:

 

1 – Officers. This has been pulled from the VISION mobilising system and
details Area Manager names and above only. Names below this level have
been redacted by applying the S40 exemption in respect of it being
identifiable personal information.

 

2 – Appliances. As above, this information has been pulled directly from
the VISION mobilising system. Call Signs and locations have been redacted
by applying the S36 exemption for which you have already been sent a copy
of the Public Interest Test.

 

Both these attachments were originally withheld under S40 and S36
respectively, however upon review, they both contain information that can
be released and is now attached.

 

3 – Seconds. This information contains the missing ‘seconds’ from the
original information sent to you that was included within the 1287 page
document that detailed the ‘narrative’ information from the Fire Control
Log and has been pulled directly from the VISION System. Please accept my
apologies for this omission in the information you originally received.
Due to the size of the incident, the usual Fire Control Log Report that
can be exported from the system failed and we were limited to producing
the first 49 pages only. As an alternative approach, we exported the data
directly from the VISION system into Excel which formatted the information
into 4 tabs:

 

-       Incident

-       Narrative

-       Appliances

-       Officers

 

The narrative tab consisted of approx. 76,000 entries and when saving into
PDF format (for the purposes of redaction under exemptions S36 and S40),
we did not realise that the automatic default for the time column was set
at dd/mm/yy hh:mm. If we had reformatted this to add :ss, the seconds
would have shown and copied into PDF. For the purposes of getting this
information to you as soon as possible, we have reformatted the time
column of the excel spreadsheet which can be reconciled to the 1287 page
PDF document previously disclosed to you.

 

The ‘entry’ column in the narrative tab has also been reviewed and where
all names were previously redacted, we have un-redacted the names of Area
Managers and above. This has been re-saved into PDF and will be posted to
you as we did previously, due to the size of the document, we cannot send
it by email. Please note this latest copy of the narrative tab will still
include the time information in its original form (minus the seconds). For
the full timeline including the seconds, please refer to the 3^rd
attachment entitled ‘Seconds’ as detailed above.

Thank you for your email below regarding the yellow highlighted text that was included in the disclosed Event & Decision Log (EDL).

As you are aware, the disclosed EDL was a copied typed transcript from 15 handwritten booklets. Upon review, I can confirm that the translator / author of the transcript had highlighted specific text that required clarification on their interpretation of various forms of handwriting to ensure the copy that was being created was true and correct.
