The PHSO holds information relevant to your request. Please see the attached spreadsheet for details.

Please note that during the calendar year 2016/17 the PHSO changed the records management system it uses for casework from the application Visual Files to Microsoft Dynamics. The outputs for these records management systems are different in some areas, so the information provided shows the numbers for each system rather than one combined total.
