I would like to know the following

Who is the author of the document. i.e.the person within Liverpool City Council or an outside agency.

If an outside agency what fee's were paid.

If it was an outside agency, who within Liverpool City Council past the document for public use.

Who within Liverpool City Council thought - Question Q12 13 14 15 suitable for a carers assessment.

How would finding out a carers sexual orientation be useful to Liverpool City Council in a carers assessment and what will they do with this information.

Acknowledgement of your request for information.
Thank you for your recent correspondence requesting information from the
City Council.  Your request was received in our offices on 08 May 2012
and, is being processed in accordance with the Freedom of Information Act
2000.

The information you have requested can be summarised as follows:

Liverpool City Council have issued a Carers Self Directed Assessment.

I would like to know the following

1) Who is the author of the document. i.e. the person within Liverpool
City Council or an outside agency.

2) If an outside agency what fees were paid?

3) If it was an outside agency, who within Liverpool City Council past the
document for public use.

4) Who within Liverpool City Council thought - Question Q12 13 14 15
suitable for a carers assessment.

5) How would finding out a carer’s sexual orientation be useful to
Liverpool City Council in a carer’s assessment and what will they do with
this information.

6) Did unpaid carers who the documents is aimed at have any input in
producing this assessment/document.

If the above summary is incorrect or you wish to change any details, let
me know at the earliest opportunity.

A response will be sent to you as soon as possible and in any case within
the statutory deadline which is 20 working days from the working day after
your request was received.

For your information, the Act defines a number of exemptions which may
prevent the release of information you have requested. There will be an
assessment and, if any of the exemption categories apply, the information
will not be provided to you.  You will be informed if this is the case

2 How long have adult social care been using this documents as a unpaid carers assessment.

3 How many unpaid carers have completed this document as a carers assessment 20010/11/12.

4 How does adult social care feel this document/assessment benefits the unpaid carer.

5 Who gave permission for this document to be used as an unpaid cares assessment

How many unpaid carers are registered with Adult Social Care.

How many unpaid carers have received a carers grant -2009/10 2010/11 2011/12 to date.

2. How much of this carer’s grant was allocated directly to carers or given to external organisations with a remit of supporting carers?
3. How many carer’s assessments were carried out in 2010/11?
4. How many carer’s assessments identified a need that led to the carer receiving regular direct payments and/or a personal budget?
5. How many carers’ assessments identified a need that led to the carer receiving a one off payment to meet that need?

Why do Liverpool City Council feel the need to include within the assessment form question(Q70)about Mental Health Wellbeing. This question is copy righted to NHS Scotland, University of Warwick and University of Edinburgh 2006.

Can you please produce the application request to NHS Scotland, University of Warwick and University of Edinburgh who owns the question asking permission for Liverpool Council to use it plus copy of contract

The carers strategy is out of date and needs to be updated with urgency.
