Please disclose communications during 2021 between EHRC; CQC; the NHS; Local Authorities and Care Homes about Equality and Human Rights issues around rapid discharge from hospitals into Care Homes as we exit the pandemic.

Where relevant, please include what the National Pensioners Convention letter to MPs described thus:
The Health and Care Bill removes the requirement for social care needs assessments to be carried out by the relevant local authority before a patient is discharged from hospital; this will leave families to pick up the pieces, and those without family at risk of isolation and lack of care.

EHRC have no directly relevant information but in 2020 reported critically to MPs on equality and human rights in care homes. i asked what response they had had and they are treating this question as a new FOI request.

Though my original request was about human rights [incl informed choice] about immediate hospital discharge and choice of care setting beyond the pandemic , EHRC assert that they have no information. However they have provided a link to a 2020 Parliamentary briefing roundly criticising the governments handling of care during the pandemic.
i have asked EHRC about responses to the briefing which they are treating as a second FOI request under the same reference.
