Care funding under the PCT

Response to this request is long overdue. By law, under all circumstances, Bolton Teaching Primary Care Trust (PCT) should have responded by now (details). You can complain by requesting an internal review.

Dear Bolton Teaching Primary Care Trust (PCT),

Please answer the following;

1. How many patients under the PCT were eligible for continuation of care funding between 2010-2011

2. What percentage (%) of these patients, allocated funding by the PCT, continued their care by being supported in their own home i.e. not residential

3. What was the total expenditure that the PCT allocated to
continuation of care between 2010-2011, for patients who were eligible, to be cared for within their own home

4. How does the PCT assist patients in identifying and accessing the best social care and nursing partners to support the patient in their own home. What is the process?
