Care funding under the PCT

1. How many patients under the PCT were eligible for continuation of care funding between 2010-2011
2. What percentage (%) of these patients, allocated funding by the PCT, continued their care by being supported in their own home i.e. not residential
3. What was the total expenditure that the PCT allocated to
continuation of care between 2010-2011, for patients who were eligible, to be cared for within their own home
4. How does the PCT assist patients in identifying and accessing the best social care and nursing partners to support the patient in their own home. What is the process?

We refer to your recent enquiry and are now able to respond as follows.
1. How many patients under the PCT were eligible for
continuation of care funding between 2010-2011
Cumulative activity for ONEL PCT at Q4 for 2010/11 is 1676
2. What percentage (%) of these patients, allocated funding by
the PCT, continued their care by being supported in their own home i.e.
not residential
It is not possible from the systems we use for us to define a percentage.
3. What was the total expenditure that the PCT allocated to
continuation of care between 2010-2011, for patients who were eligible, to
be cared for within their own home.
We do not hold the information required in the format of a patients
“site”. It would be necessary to investigate every single file to
identify the information required. We do not have the capacity to do this
and it would inevitably exceed the 18 hours limit for FOIs.
We are however able to provide the overall costs for your reference.
The continuing Care expenditure for the four ONEL PCTs combined for
2010/11 was £44,913,000.
Our estimated continuing care expenditure for the four ONEL PCTs combined
financial for 2012/13 is £49,683.659
4. How does the PCT assist patients in identifying and
accessing the best social care and nursing partners to support the patient
in their own home? What is the process?
The work of the individualised care team is person centred and this
informs everything that we do. When a patient qualifies NHS continuing
healthcare, the nurse assessor in partnership with the patient and their
support network will identity the patients needs.
In partnership with the patient and their representatives the nurse
assessor will procure a care package to support the patient’s individual
needs.
We trust the information we have provided will prove to be of assistance
to you.
