I would be grateful if you can respond to this Freedom of Information (Scotland) Act request for the hospitals in your Health Board regarding insertion of cardiac pacemaker devices.

For your: cardiology, clinical governance, finance departments
Request: any already calculated data in the form of summary reports, audit reports, research reports or held information which may help respond to the questions below.

Question 1: Can you provide the insertion numbers for basic bradycardia pacemakers (i.e. single and dual chamber pacemakers) at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (where you have recorded this data)
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 2: Can you provide the insertion numbers for implantable cardiac defibrillator (ICD) devices at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (if you have record of this data)
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 3: Can you provide the insertion numbers for cardiac resynchronisation therapy devices (CRT) at your centre?

Time Period: Can you provide this data for the years 2011 to 2017. Can you provide this for the individual years. (if you any record of this data)?
• If this data is not recorded over a calendar year (eg. Jan-Dec 2011) but instead over a fiscal or training year can you still provide this.

Question 4: Can you provide the overall complication rate for implantation of these devices?
• Complication rate for simple bradycardia pacemaker devices
• Complication rate for ICD device implantations
• Complication rate for CRT device implantations
