Under the Freedom of Information Act 2000, I request information regarding the role and actions of the Caldicott Guardian within your organization in preventing avoidable and foreseeable harm. Specifically, I request the following:

1. Records of any instances where the Caldicott Guardian was notified of potential or actual harm related to data breaches, confidentiality, or other information governance concerns.

2. Steps taken by the Caldicott Guardian in these instances to mitigate or prevent harm.

3. Any documented evaluations or investigations into failures by the Caldicott Guardian to prevent avoidable, foreseeable harm within the organization.

If this request exceeds the cost limit, I would appreciate guidance on narrowing its scope.
