Dear NHS Central Manchester Clinical Commissioning Group,
I am writing to make an open government request for all the information to which I am entitled under the Freedom of Information Act 2000. I require information on NHS buy back services.
A) Do you offer Speech and Language Therapy services which are sold to schools directly?
B) If the answer to question ‘A’ is yes, please could you provide a copy of the marketing brochure for the services provided? If it is a separate document, please could you also provide a price list for the services provided?
C) Do you offer Occupational Therapy services which are sold to schools directly?
D) If the answer to question ‘C’ is yes, please could you provide a copy of the marketing brochure for the services provided? If it is a separate document, please could you also provide a price list for the services provided?
E) Do you offer Physiotherapy services which are sold to schools directly?
F) If the answer to question ‘E’ is yes, please could you provide a copy of the marketing brochure for the services provided? If it is a separate document, please could you also provide a price list for the services provided?
G) If the answer to question ’A’ is yes, then what did you charge schools for Speech and Language services during the last financial year (2015/16)?
H) If the answer to question ’C’ is yes, then what did you charge schools for Occupational Therapy services during the last financial year (2015/16)?
I) If the answer to question ’E’ is yes, then what did you charge schools for Physiotherapy services during the last financial year (2015/16)?
I would like the above information to be provided electronically. If any of this information is already in the public domain, please can you direct me to it, with page references and URLs if necessary.
