Dear Cambridgeshire County Council,

In your report 'Investigation into the derailment on Cambridgeshire Guided Busway to the north of Trumpington Cutting 22nd February 2016', section 3C, page 20, you say 'The considerable damage to the upstand further along the beam shown in Section C Photograph 5 is consistent with damage caused to the guideway elsewhere on previous occasions where a guide wheel has sheared off'.

Can you please provide counts, dates, locations and further descriptions of the 'previous occasions where a guide wheel has sheared off'?
