Dear Health and Safety Executive,

I am writing to make an open government request for all the
information to which I am entitled under the Freedom of Information
Act and the Environmental Information Regulations.

In order to assist you with this request, I am outlining my query
as specifically as possible.

If however this request is too wide or too unclear, I would be
grateful if you could contact me as I understand that under the
act, you are required to advise and assist requesters.

I refer to the incident which occurred in Brunswick Park, Wednesbury Sandwell West Midlands on 5th November 2014, involving several people injured at a fireworks display organised by Sandwell Council and Jubilee Fireworks.
The council's own internal investigation was only recently put in the public domain, via a Freedom of Information request at Whatdotheyknow.com, and paints a different story to comments made in the local newspaper at the time by senior councillors who blamed the injuries on "a stray firework".

(i)Can I therefore request if Sandwell council have reported this matter to yourselves or is it the responsibility of those injured parties to make a complaint concerning Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 1995 RIDDOR?
(ii) Are you investigating this matter under the Health and Safety at Work Act 1974, or other relevant legislation?
(iii) If yes can you please release your findings/report or if not completed reveal at what stage your investigation is currently at and how long this may take.

I understand that under the act, I should be entitled to a response
within 20 working days.
I would be grateful if you could confirm that you have received this request. I look forward to hearing from you in the near future via the what do they know website address.
