Dear Department of Health,

On 3rd December 2010 the Chief Executive of University Hospitals Bristol NHS Foundation Trust emailed Monitor saying:

"(FT status notwithstanding) I do not have clearance from DH yet to
publish on Wednesday 8/12 but the informal feedback is to keep
going for that date".

Please provide me with the following information:

1. The documents used by the Department to make a decision on clearing the report for publication on 8th December 2012. I assume this to be some sort of briefing pack.

2. Communications (including any briefing pack) between the Department and Verita regarding the final version of the report and the release date.

3. According to the ICO, the Department has found documents relating to the Histopathology Inquiry in the following places. I would like copies please:

2 in David Nicholson's office
6 in Press Office
7 in Professional Standards Division
