Break Down of Community Mental Health Teams

Response to this request is long overdue. By law, under all circumstances, Lincolnshire Partnership NHS Foundation Trust should have responded by now (details). You can complain by requesting an internal review.

Dear Lincolnshire Partnership NHS Foundation Trust,

Good Morning,

I am writing to you in request of a break down of all the community mental health teams run by the trust as I seek to gain a better understanding of the services offered and the types of nurses utilised in each of these teams.
Whilst I am only really interested in community teams – a complete service map would also be helpful to me.
