Blocking of email accounts additional clarification

Thank you for the information provided to date. I would appreciate further clarification on the information provided by you:

1. How many of those email accounts blocked were authorised by the DITSO?
2. As DITSO authorization is an official action which needs to be documented for audit purposes please specify the numbers authorized in 2010, 2011 and 2012
3. How many of those email accounts blocked were individuals rather than domain names?
4. How many of those email accounts blocked relate to non spam related issues?
5. When the email accounts are blocked are you required to notify the email account user?
