Dear General Register Office,
Would you please explain the purpose of the additional yellow or pink information sheet that accompanies a supplied request for a copy birth certificate?

Specifically the GRO Reference information and why it is completed very rarely?

For example my particular copy issued 27th Sep 2012 - 4363151/1 - contains hand written notes such as "poss" 1953 M Camberwell 5c 209
Entry year 1952
D/S Year 1953
Yes Year 1954
all of which is initialled 27/9 ints.

Dear Mike,
 
Thank you for your enquiry.
 
I have handled your request as official correspondence rather than
under the provisions of the Freedom of Information Act, but this has made
no difference to the content of my response.
 
The sheet that accompanies a certificate is the application form produced
from the certificate ordering system which has your address and order
details on it.
 
The GRO Reference Information is for office use only; for example if you
have not supplied us with a GRO index reference we will try to locate the
entry from our records and complete the relevant boxes with a possible
match.

The answer you provided did not fully relate to my questions.

I need to know what the hand written entries at the bottom of the yellow sheet/page indicate and/or confirm?

Also could you confirm where the details highlighting Mothers Maiden name etc, are obtained and which, in this case are different from the mothers maiden name on my actual birth certificate?

Are you aware that all future such pink and yellow forms do not include those particular boxes "Mothers Maiden Name" & "Mothers Forename"... Is there an official explanation for this

Dear Mr Newby,
 
As previously advised the hand written notes at the bottom are for office
use only, this helps us locate the entry from our records and complete the
relevant boxes with a possible match. Therefore the information at the
bottom of your application is a possible match to the entry located.
 
In regards to obtaining the Mothers Maiden name, this will appear in the
online indexes however in some cases Mothers Maiden Name was not data
captured, and will appear in the online indexes as a dash.
 
I can confirm that the fields ‘Mothers Maiden Name’ and ‘Mothers Forename’
appear where you have specified it on your application.
 
We are unable to help you further.
