Being stopped/questioned by police while driving or stationary

Dear West Midlands Police,

I am writing to request information under the Freedom of Information Act 2000. The request is outlined below in the most precise way possible. The request is in regards to being stopped by police while driving or being questioned by police while stationary.

1. Is the driver legally obliged to provide name, address and date of birth, or any other information whenever a police officer requests for this at road side?
2. Does the driver have right to refuse to provide such information under any circumstances, if not, what are the consequences if a driver refuses to do so, if any at all?
3. Do police officers keep a any form of record of the conversations and actions even when no crime has been committed, if so, could you provide an example of such report? Could you also outline what is contained within the report.
3.1 Can the driver request for this report at some point in future, if so, what is the process for requesting this report?
3.2 Is this report accessible by any other police officers e.g. can this report be viewed by another officer at a different date if pulled over again for same reason or for another reason?
3.3 Is the report recorded under the vehicle reg. or driver, or both?
3.4 How long is this report retained on the police system and accessible by officers?

Please be advised that Section 8(1)(b) of FOI legislation indicates that the real name of an applicant should be used when requesting information and not any other name, for example, a pseudonym.

Title and/or first name, or first name initials, with surname satisfies the requirement for provision of a real name and you have provided a title and second name, but in order to validate this and proceed with your request we will require proof of this name please.

Should you wish to proceed with this request please provide formal proof of your name, scanned and e-mailed, to the FOI Unit. This is for validation purposes only, in order that we can ascertain that your request is valid under Section 8(1)(b) of FOIA.

After receiving your reply, your request will then be considered and you will receive the information requested within the statutory timescale of 20 working days as defined by the Freedom of Information Act 2000, subject to the information not being exempt or containing a reference to a third party.

Who decides whether Mohammed is my first or last name??

Mr. is my title, Mohammed is my first name and Ahmad is my last name, thought this was accessible to you via my profile and the obvious was not to be indicated. This satisfies FOI requirements, please proceed without any further delay or I would have no choice but to escalate for an internal review for failing to comply with FOI request, making false assumptions about ones name and accusing one of providing false name without any evidence.
