Dear NHS North Somerset Clinical Commissioning Group,

I understand you are LEAD ? / co commissioners of the Avon & Wiltshire Mental Health Partnership, and are participants in the QIG Quality Improvement Group that is in place to control,monitor and encourage improvement of MH services at AWP.

I have read the group has concerns regarding the under reporting of Serious Incidents, along with other matters.

I understand the QIG meets infrequently.

I am interested to see all the information you hold, say over the past five years, where various commissioners have met to discuss concerns regarding the AWP, and how the matters have been identified, assessed , and dealt with.

I understand the QIG is one of a number of investigative groups the Commissioners have put in place, and wish to also be made aware of all the others in respect of AWP.

I am interested in seeing the information supplied in at leat two sections please, as I have a mental health injury and am disabled and need to ask for a ' reasonable adjustment ' please:

1. I wish to see all the information relating to ' outpatients in the community '

and although most Mental Health patients are in the community, it appears most of the attention is directed to ' in patient hospitalisation ' where I understand only some 10 - 12% of patients receive their care -

2. The information relating to ' inpatients in hospitals '.

I would be interested to see, how the under reporting of deaths of mental health patients in hospitals, and in the community , is being dealt with jointly by Commissioners, along with the under reporting of patient deaths to Coroners within the AWP service areas, as featured in HSJ and national press this week ?

Would you also be kind enough, under FOIA , to provide me with all of the agendas, minutes , notes , reports seen and prepared by QIG and other investigative groups comprising Commissioners acting jointly, including information from CQC and other regulatory bodies along with the action plans and other remedial actions QIG and others have requested be put in place.

Who attends these meetings, and what is the ' real ' progress being made ?

I am also interested to receive information regarding the information you hold regarding AWPs Safeguarding of Vulnerable Adults , which I understand was subject to a number of scrutiny group meetings a number of years ago, and trust is ongoing ? May I please see these findings for the period 01 01 2014 up to date please?
