Dear NHS Wiltshire Clinical Commissioning Group,

You have been commissioning services from the Avon and Wiltshire Mental Health Partnership NHS Trust for some 2.5 years .

My request is in part prompted by the AWP Action Plan following the recent PHSO Judgement which states " Attention needs to be given to the development of appropriate professional behavior in the handling of complaints . This includes honesty and openness and a willingness to listen to the complainant, and to understand and work with the patient to rectify the problem ".

Would you please let me have a FULL copy of the investigation by the PHSO - not just the summary that appears somewhere on the PHSO website - I trust you hold and are fully aware of the very serious investigation ?

The PALS and Complaints team members have been in post for many many years, indeed I believe the manager worked for a number of years with NHS Wiltshire PCT handling complaints ?

I therefore believe , not only from my own bad experiences with AWPs complaint handling processes but also from what I have heard from others and read about , the findings of the PHSO investigation are not an isolated incident but an ongoing long term systemic failing ?

Will you please let me see * ALL * the information you hold where you have carried out checks , inspections , audits , attended their Board Meetings to ask questions and/or raise concerns, where you have requested reports or information from them regarding matters related to service standards and compliance with Data Protection / Health Records / and associated NHS Legislation and requirements / guidance issued by the NHS ?

May I also see * ALL * the contacts from AWP to yourselves over the same period where they have contacted you to inform you they have failed to meet the required standards in the areas I am enquiring about ?

May I also see all the reports you hold about AWP that you have as prepared by the CQC , Parliamentary Health Service Ombudsman, NHS England or any other NHS body, including all contacts from any other commissioner of services from AWP where they have contacted you in respect of any irregularities or concerns they have become aware of ?

Would you also be kind enough to supply me with duplicate copies of * ALL * similar documents received from NHS Wiltshire PCT and their staff, including those that transferred to your CCG when you took over the commissioning responsibilities for AWP ?

I note a number of other FOIA requestors using the whatdotheyknow website ALWAYS receive an automated acknowledgement followed by a personalised one from the SWCSU,
and ask you to fully explain why I appear to be the only one not receiving a similar service to others using this site?

Please issue a reference number for this request with your acknowledgement.

It appears this long overdue request is ' lost in the long grass ' , although it was submitted when the grass was short .

Do you intend to comply with my FOIA Rights in Law , or continue demonstrating how well as a senior representative of the NHS in Wiltshire you treat this vulnerable patient living in your area ?

Despite also sending this request AGAIN last August 2016,
by signed for recorded delivery , it never received any response.
Has the FOIA changed to remove the Legal Rights of a Wiltshire patient to make a request and receive information under FOIA , or are there still unresolved issues at NHS Wiltshire CCG that are having a deliberate negative effect on patients seeking information ?
