Dear NHS Greenwich Clinical Commissioning Group,

Could you please provide the statistics for the median mortality age of Autistic people within the Borough for the last 5 years.

Could you please provide the statistics for the number of autistic people and people with Learning disabilities who are identified as vulnerable adults within the borough within the last 5 years.

Could you please outline the processes and procedures within Greenwich CCG and the Local Authority once someone is identified as a vulnerable adult and/ or a safeguarding concern has been raised and the number of individuals (adult autistics) who then receive formal treatment and/ or social support following on from a person being identified as a vulnerable adult and/ or a safeguarding request being made from 2016-present.

Could you please outline the procedures and contact details of individuals involved with the administration and decision making process for an autistic adult applying for a personal healthcare budget.

Could you please provide a copy of Greenwich CCG's guidelines and criteria for considering an application for a personal healthcare budget for an autistic adult within the borough.

Could you please provide the statistics for the number of autistic adults who are identified through safe guarding requests as requiring additional support, the number who receive a care assessment and subsequently the number of those people who are able to obtain a care assessment (numbers at both initial and advance care assessment stages) who receive a personal healthcare budget within the last 5 years.

Could you please provide the contact details (including email addresses and direct telephone numbers ) and names of the individuals on the Autism Strategy Board.

Could you please provide the Autism Strategy Board's current Autism Strategy and strategic goals 2019 and for the next 3 years.

reasonably require clarification from you on the scope of your request.
Freedom of Information Act 2000, Section 16 stipulates a duty for a public
authority to provide advice and assistance when they receive an unclear or

Please could you define care assessment type? Is it continuing healthcare
assessment or a local authority care assessment?
