Dear Mr Wright

I am writing to confirm that we have now completed our search for the
information, which you requested on 22 September 2020.

Please find attached a copy of the incident log for your attention.

Taking your questions in order:

The cause of the incident - Causation opinion and subsequent data for
collisions is formulated by and disseminated by the relevant police force
for the area. You would need to speak with Avon & Somerset Police to
obtain this information.

Cost of the incident to Highways England – The total cost was £4,565.77.

We usually delete our CCTV footage after seven days unless there is a
specific requirement to keep it. Due to an administrative error on our
part, we failed to arrange for the footage to be kept following your
request.  As the police didn't request the footage for investigative
purposes, any footage we may have held related to the incident was deleted
seven days after it was recorded. Please accept my apologies for any
frustration this may cause.

Most documents supplied by Highways England will be Crown Copyright. Most
Crown Copyright information can be re-used under the [1]Open Government
Licence. For information about OGL and about re-using Crown Copyright
information please see [2]The National Archives website.

If you are unhappy with the way we have handled your request you may ask
for an internal review. Our internal review process is available at:

[3]https://www.gov.uk/government/organisati...

If you require a print copy, please phone the Information Line on 0300 123
5000; or e-mail [4][Highways England request email]. You should contact me if
you wish to complain.

If you are not content with the outcome of the internal review, you have
the right to apply directly to the Information Commissioner for a
decision. The Information Commissioner can be contacted at:

          Information Commissioner’s Office

          Wycliffe House

          Water Lane
