Below is a copy of the letter I sent to Judith Paget (ABHB) and Kate Chamberlain (HIW) dated 22 November 2016

Both the Chief Executives have ignored the evidence of violations of patient's medical records.

Thus, the reason for publishing the letter of the 22 November 2016, below.......

Dear Judith Paget - Chief Executive of Aneurin Bevan Health Board /

Kate Chamberlain - Chief Executive Health Inspectorate Wales

RE: In relationship to Rhys Jones’ correspondence dated 16 November 2016 and the attached letter from Judith Paget dated 14 November 2016.

1. Considering the Aneurin Bevan Health Board (ABHB) / Judith Paget are unprepared to be transparent and honest regarding the illegal accessing of my (Miss Wendy Williams) health records and my brother’s (Mr Mark Williams) health records that are held by the ABHB / NHS and the HIW / Rhys Jones are unprepared to investigate the negligence on behalf of the ABHB. My family are giving the ABHB and the HIW the ultimatum, that either we deal with the three cases I have brought to the HIW (which was relayed to the ABHB by the HIW) or my family and I will bring the negligence of the ABHB and the HIW to the public's attention.

2. Judith Paget has been creative and obstructive with the ABHB’s responses to the complaints and never producing hard fact to substantiate the ABHB claims.

Below is a partial list of the illegal accesses into Mark Williams’ records held by the ABHB / NHS. I have also attached, (contained in the evidence file), copies of the records supplied by Head of Information Governance Richard Howells, who has been aiding the ABHB in covering up of the ABHB’s negligence. (I have removed the other accesses into Mark Williams's records, as to evade confusion.)

To prove the above documented lists are illegal accesses, I am providing a partial copy of an official police investigation report, produced by your partner agency Gwent Police, Standards Department. Gwent Police have confirmed officially that they have ascertained that my brother Mark Williams, “has never been detained or arrested”. Therefore, the documented lists above are all illegal accesses into my brother health records, held by the ABHB. As Forensic Mental Health pertains to:-

Forensic mental health services are specialist services for people who have a mental health problem who have been arrested, who are on remand or who have been to court and found guilty of a crime. ‘Forensic’ means ‘pertaining to the law’.

Should the ABHB cite the usual excuse that the job titles recorded, is the individuals current job tile and not the job title when they accessed my brother’s records. I bring to your attention the letter from Richard Howells, I obtained via the Information Commissioner’ Office (dated 12 March 2015) which clearly states:-

“She is currently employed as a Social Worker Secretary but at the time was part of the Forensic Team.”

Hence the Forensic Team illegally accessed my brother’s health records. The Forensic Mental Health Team knew exactly who records they wanted to access, as they accessed the Patient Master Index and located my brother’s records.

NHS states:-

“The Patient Master Index is a system that makes it easier to ensure patients are correctly identified and minimises the number of duplicate health records held for each patient and maintain a definitive source of patient demographic data for each patient (name, address, date of birth, sex.”

Thus at this point, the Forensic Mental Health Team on reading the Patient Master Index information would have eliminated or identified the individual the Forensic Mental Health Team were seeking.

On identifying the individual, the Forensic Mental Health Team were looking for, in this case my brother Mark Williams. The Forensic Mental Health Team then proceeded to illegally manipulate my brother's records. This included emailing, editing contacts, editing referral, editing registration details. ALL illegal actions that the ABHB are turning a blind to.

I now refer to the illegal accesses that occurred with my, W Williams’s health records held by the ABHB / NHS.

It can be clearly seen from the attached letters (contained in the evidence file), I Miss Williams was seen by Dr P Tudball in 14 November 2006, who documented in her report, that the local authorities and their informants were attempting to discredit my brother and make it hard for my family to obtain justice for the crimes committed against my family. Thus I, Miss Williams suffered with anxiety etc., due to the ongoing campaign of harassment, victimisation and other criminal acts committed against myself and my family, by the authorities and their associates. On 10 April 2007, Liz Galle Occupational Therapist wrote a letter (contained in the evidence file) to my doctor, stating “Wendy Williams has now completed a course of Anxiety Management”, “Wendy will therefore be discharged from the Day Unit”.

It can also be seen from the addition attached letter that, on the 22 March 2013 (contained in the evidence file), my GP wrote to the ABHB stating, “Wendy has been seen by FAST and by Dr Pat Tudball in 2006. She was then referred to Pam Lyons at the day hospital for some relaxation and possibly anxiety and management.”

Hence, giving a leeway for a delay in ABHB staff doing official paperwork. There should have been no accessing into my health by the Mental Health Department, between May 2007 and March 2013. Yet, it appears that the Mental Health Staff have been instructed by someone or abusing their position, by illegally accessing, editing, distributing my health records, for someone’s gains.

Below is a partial list of the illegal accesses into Wendy Williams’ records held by the ABHB / NHS. I have also attached, (contained in the evidence file), copies of the records supplied by Head of Information Governance Richard Howells, who has been aiding the ABHB in covering up of the ABHB’s negligence. (I have removed the other accesses into my, Wendy Williams's records, as to evade confusion.)

I am aware from the information the Access to health records department - ABHB provided, in response to my Subject Access Request, made at the beginning of 2016, that certain individuals that have access to ABHB / NHS records have either hidden or removed certain medical information relating to my health care. My belief is that my missing medical care history did not correspond to the false mental health history that the individual(s) have attempted to plant with my ABHB medical records. Hence, the reason the Access to health records department did not forward ALL my medical records from the mental health department.

It is noted that the ABHB and the HIW have refrained from commenting on the other patients records the ABHB supplied to me, in response to my Subject Access Request, in 2016. Why are neither of you (ABHB or HIW) concerned of the breach of patients private medical data, released by the ABHB?

I am hereby giving you the Aneurin Bevan Health Board and the Health Inspectorate Wales, 2 working days to separately acknowledge receipt of this correspondence and 7 working days for full separate responses from you both, detailing the actions your organisations intend to take regarding these 3 separate cases.

Should no actions be taken regarding these separate cases, I have been given permission to release details and the evidence of the ABHB's negligence, into the public domain. For the public to be made aware of how the authorities are illegally using certain people's personal / medical records to discredit individuals and victimising the patients by withholding their treatment, etc.

I await an acknowledgement from you, Judith Paget and you, Kate Chamberlain to this correspondence. Additionally, the ABHB and the HIW's intended actions regarding these cases.
